Fees Doctor Gammer's fees are based on what is standard for the community and reflect the complexity and quantity of time needed for a specific treatment.


Billing and Payments First office visit payment is required at the time of the appointment. Acceptable methods of payment include: Cash, Check, American Express, Discover, Visa� or MasterCard�. We will be happy to assist you in billing your insurance carrier. Insurance deductibles and co-payments are due at the time services are rendered. Payment plans may be set up to fit your economic needs. HMO patients are welcome, with special financial arrangements.


Insurance Bring insurance information for primary and secondary insurers with you at the time of the appointment. Insurance coverage differs from company to company. Dr.Gammer is a member of most PPO plans and is a Medicare Provider. If the procedure is not covered by your insurance, we will be happy to work out a payment plan.


Prescriptions / Renewals We would appreciate all prescriptions, refills, and authorizations be requested during normal office hours during weekdays, Monday through Friday.
